Making the plans concrete
A maximum of two weeks after drawing up your personal description, we start working on the planning. We work on the support plan in your care folder. With the planning, we make the approach concrete.
Together with your counsellor, you determine which goals for change or stabilisation you will work on. To do this, we use the information from the previous phase(s).
Draw up a guidance plan
Your personal description is an important starting point for the goals of our assistance. We lay down the approach and planning in the support plan.
The plan is based on your needs. Together with us, you sign the support plan that we draw up, including the planning that goes with it. This includes both the concrete plans and the period in which you want to achieve these goals.
