Outflow
Goals:
- Concluding and/or handing over assistance
- Evaluate guidance
- Returning to society
Preparation, follow-up, and transfer
Before the outflow, we prepare the conclusion together. During counselling sessions, we discuss expectations for the new period and evaluate the steps taken.
Together, we will discuss whether you want follow-up assistance. Using the social map, we discuss the available options. By applying for follow-up assistance in time, we can avoid any waiting lists. In addition, we arrange a number of practical matters, such as administration and finances.
Finally, a few days before leaving, we hold the exit interview. We evaluate and reflect on the total guidance and what has been achieved. In addition, we pay attention to what went well and what could be improved. This helps us to further improve the guidance.
Voluntary or unplanned resignation
We assume successful supervision and voluntary outflow. Please note that outflow is also the result of an unplanned dismissal. In that case, we may not be able to carry out all the activities (properly).
