Evaluation interview with your supervisor
During the evaluation phase, you will talk to your supervisor. We will discuss the goals set and whether they have been achieved. We will zoom in on the specific actions, and reasons why they were successful or not.
You have the opportunity to discuss the cooperation and to indicate what is going well or what needs to be improved. Moreover, this is the moment when we determine whether you are still well on your way to your final goal.
Criteria for evaluation
We evaluate and use a number of important criteria for this purpose. These include:
- Results achieved so far
- Motivation of you as a client
- Cooperation with care providers
- Your request and our offer
Outcome of the evaluation
The evaluation leads to one of roughly two decisions:
- Completion and outflow
In case of good results or lack of them, we can decide to complete the programme and move on. This starts the outflow phase (phase 8). - Continuation of guidance
Your counselling continues. We restart the counselling cycle. We use information from the implementation and evaluation phase to clearly identify your strengths and weaknesses. On this basis, we will draw up a new support plan and a new implementation phase will begin.
